Outdoor Permission Slips and Registrations 2025

Permission Slips for Open Activities: Required for ages 12-18. Ages 11 and under must be accompanied by a parent or their parents must arrange for another adult to supervise them during the activity. Available transportation and gear may limit the number of unaccompanied minors who can participate.
Registration/Permission for Restricted Trips (Outdoor School registration is porcessed by AGA, Pathfinder Campout registration is processed by Pathfinder's)

I understand that participation in activities and trips organized by Amazing Grace Academy and the Palmer SDA Church may involve inherent risks, including the risk of injury. In consideration of my child’s participation, I hereby release, indemnify, and hold harmless Amazing Grace Academy, Palmer SDA Church, their staff, volunteers, and representatives from any and all claims, liabilities, or damages arising from my child’s participation, except in cases of gross negligence or willful misconduct. I give permission for my child to participate in all scheduled activities, including overnight trips.
