There is a lot going on in the following form. It is one form for all our various outdoor permission slips and registration. Since it is not customized to each individual trip, use the comment section to address trip specifics.

Outdoor Permission Slips and Registrations 2026

Permission Slips for Open Activities: Required for ages 10-18 without a parent present. Ages 9 and under must be accompanied by a parent or their parents must arrange for another adult to supervise them during the activity. Available transportation and gear may limit the number of unaccompanied minors who can participate.

I understand that participation in activities and trips organized by Amazing Grace Academy and the Palmer SDA Church may involve inherent risks, including the risk of injury. In consideration of my child’s participation, I hereby release, indemnify, and hold harmless Amazing Grace Academy, Palmer SDA Church, their staff, volunteers, and representatives from any and all claims, liabilities, or damages arising from my child’s participation, except in cases of gross negligence or willful misconduct. I give permission for my child to participate in all scheduled activities, including overnight trips.
